A 5% fee is charged to the client’s saved payment method after successful claim submission. It is calculated on the amount billed to the insurer, not the reimbursement received.

When the fee is charged. The fee is charged once, after a claim is successfully submitted — not when the insurer pays, and not on the amount reimbursed. A replacement claim that supersedes a claim whose fee was already charged is not charged again.

Refunds. If a claim is denied, the 5% fee can be refunded. Refunds are not automatic: a refund has to be requested against the claim, and a practice admin reviews and decides it. A claim applied to the client’s deductible does not qualify for a fee refund.
